One of the main reasons that most initiatives fail is that key requirements were missed at the start when designing a solution and setting budget and schedule expectations.  Missed requirements tend to be revealed near the end of the project when there is the greatest risk of costly rework and a strong likelihood of missed deadlines and exceeded budgets.  To counter this problem, experts have created a number of practices focused on exploring stakeholder requirements to reveal these hidden aspects early on to avoid surprises late in the project.
